1095-C Notice​

Form 1095-C includes information about the health insurance coverage offered to employees by Spectra Logic Corporation (“Spectra”). Effective December 23, 2024, Spectra is no longer required to provide all U.S. employees with a copy of Form 1095-C. However, if you would like to receive a copy of your Form 1095-C, you may submit request to [email protected].

How to Request a Copy:

To request your Form 1095-C, please send an email to [email protected]. Once your request is received, your Form 1095-C will be provided to you within 30 days.

Electronic Delivery:

By submitting a request for your Form 1095-C, you agree to accept the form electronically via email. However, if you do not consent to receiving your Form 1095-C electronically, we are happy to provide you with a paper copy. If you prefer to receive the form in a paper format via mail, please specify this in your request and provide your mailing address.
